Software Installation Request Form

           

UCF's Computer Services department maintains a campus local area network (LAN) for campus academic use. Many software packages are already available on the LAN, but UCF instructors may request the installation of software that they or their department owns for their student's use.  The software must be for class-related, academic use only. If you do not receive notification confirming your request submission in 48 hours, please email cstapps@mail.ucf.edu.

Instructor agrees to:

  • Teach his or her students how to use the software and answering their questions about it.  Any questions Computer Services receives about using the software will be directed to the instructor.
  • Acquire and supply Computer Services with any modifications (upgrades, patches, fixes or amendments) to the program.
  • Before installing a program on the LAN, Computer Services requires all of the following:
    • Proof of ownership (examples include a copy of purchase order, receipt, software registration, a letter from the software vendor or distributor).
    • Software license agreement with the number of concurrent users allowed.
    • Provides installation and user instructions, as well as the software media.
    • Windows XP compatible
  • The instructor is responsible for logging into the LAN and verifying the software setup before each semester begins.


Computer Services agrees to:

  • Please allow a minimum four weeks for installation. Each request is reviewed on a case by case basis.
  • Computer Services will not release, transfer the program or any part of it to anyone other than the instructor without prior written consent of the instructor. Computer Services reserves the right to refuse to copy, release, or otherwise transfer the program or any part of it such that the action constitutes a violation of the license agreement.
  • Computer Services will remove the program at the end of each semester.  The instructor must submit a new request form to keep the program on the LAN.
  • Computer Services will return the software media, installation instructions, and manuals to the instructor after the software is installed.
  • Computer Services will install the requested software provided there are no technical problems that would prevent this or previously installed software from functioning properly.
